Please let me know the type of issue in one or two brief sentences only (e.g. low mood, relationship problems, anxiety). You should include a contact telephone number and your full name. This information will not be kept if counselling does not proceed and is mainly to ensure your issue is within my area of work. Urgent support should always be sought from your GP or local hospital first. You must be the person seeking counselling - unless you provide communication assistance.
